Waiver of Liability, Indemnity and Consent
Advance Soccer Academy
By enrolling my child into a program or activity delivered by Advance Soccer Academy, I acknowledge and agree to the following terms and conditions:
1. Assumption of Risk
1.1. I understand that football is a physical sport and participation in any Advance Soccer Academy activity involves inherent risks, including but not limited to injury, permanent disability, or death.
1.2. My child participates at their own risk, and I accept full responsibility for any injury, loss, or damage that may occur during their involvement with the Academy.
1.3. I acknowledge that Advance Soccer Academy does not provide personal injury or medical insurance cover for participants.
2. Medical Fitness and Disclosure
2.1. I confirm that my child is in appropriate physical and mental condition to safely participate in football activities.
2.2. I am not aware of any medical condition, injury, disability, illness, or other concern that may increase the risk to my child or others during participation.
2.3. I understand that I must disclose any pre-existing medical condition that may affect participation and agree to accept full responsibility if I fail to do so.
3. Medical Consent
3.1. I consent to first aid being administered to my child by Advance Soccer Academy staff who hold a current first aid certificate.
3.2. If my child becomes seriously ill or injured, I authorise Advance Soccer Academy to seek emergency medical treatment and arrange transport to hospital if necessary.
3.3. I understand that I am responsible for all associated medical, hospital, and ambulance costs.
4. Conduct and Behaviour
4.1. I agree that my child will behave in a safe, respectful, and sportsmanlike manner at all times during their involvement with the Academy.
4.2. I acknowledge that the Academy reserves the right to remove my child from any session or program if their behaviour is deemed dangerous, disrespectful, disruptive, or inappropriate.
4.3. The determination of such behaviour will be made at the sole discretion of Advance Soccer Academy staff and I agree to be bound by that decision.
5. Drop-off and Collection
5.1. I give permission for my child to be collected from the Academy by nominated contacts only.
5.2. I will notify Advance Soccer Academy immediately if there is any change to authorised collection details.
5.3. I understand that children will not be released into the care of anyone under the age of 18.
6. Privacy and Media Consent
6.1. I consent to my child’s personal information being collected and stored for the purpose of program administration and communication.
6.2. I understand that this information may be used for Academy updates, enrolment purposes, and occasional promotional communication.
6.3. I give permission for photographs and videos to be taken of my child during Academy sessions or events and used for promotional and marketing purposes.
6.4. Photos or videos taken by parents or guardians must be used for personal purposes only and may not be publicly shared without consent from other families involved.
7. Parental Supervision and Safety
7.1. I will supervise any non-participating children in my care while attending Academy venues.
7.2. I understand that children who have not yet started school must be accompanied by a parent or guardian at all times unless otherwise stated.
7.3. Children may only attend holiday programs unaccompanied if they are aged 4 years and 6 months or older.
8. Venue Safety and Hygiene
8.1. I will ensure my child brings a filled water bottle to each session.
8.2. No food or drink (aside from water) is permitted on the playing area.
8.3. Smoking is prohibited at all Advance Soccer Academy venues, both indoors and outdoors.
8.4. I agree to follow all venue-specific rules and directions given by Academy staff.
9. Illness and Absence
9.1. I understand that my child must not attend any session if they are unwell or displaying signs of illness.
9.2. I agree to make alternate arrangements if my child is sick and will not send them to sessions for the safety of all participants.
Declaration
I have read and understood this Waiver of Liability, Indemnity, and Consent. I have had the opportunity to ask for clarification on any areas of concern. By submitting my child’s enrolment, I declare that I freely and voluntarily accept these terms as a condition of participation in Advance Soccer Academy programs.
